  Title: The Diagnosis of Minimal Change Disease in Diabetic Nephropathy

  Synopsis:   Heavy proteinuria is a common event in diabetic patients with kidney disease (diabetic nephropathy) and is usually associated with diffuse or nodular glomerulosclerosis and progressive renal failure. Other nondiabetic diseases may also cause proteinuria in diabetic patients and these may have a different clinical course. Renal biopsy examination, including electron microscopy, has a critical role in the diagnosis and management of nondiabetic glomerular disease in diabetic individuals with proteinuria.

      Abstract  
      Proteinuria is common in diabetic patients and usually reflects the presence of diabetic glomerulosclerosis. This paper reviews the differential diagnosis of proteinuria in diabetic patients and discusses the role of renal biopsy examination in identification and management of minimal change disease in this cohort. Identification of nondiabetic glomerular disease requires careful correlation of clinical history and renal biopsy findings and may have important implications for prognosis and therapy.
